Percutaneous nephrolithotomy (PCNL) is a procedure that is used to remove kidney stones from the body when they can’t pass on their own. A scope is inserted through a small incision in your back to remove the kidney stones. This treatment is often performed when a person has larger stones or when other procedures, such as extracorporeal shock wave lithotripsy or uteroscopy, are unsuccessful or not possible then percutaneous nephrolithotomy is done.
Percutaneous Mephrolithotomy is typically recommended if the person is suffering from following situations:
Before you undergo Percutaneous Nephrolithotomy procedure, your surgeon may ask you to get few test done that may include CT scan, ultrasound or X-ray imaging is also recommended to guide access to the kidney. This is done with a local anesthetic, meaning the patient is kept awake but will not feel any pain. The patient is later transferred to the operating room for the actual procedure.
Alternately, the surgeon may prefer to access the kidney in the operating room while the patient under general anesthesia. In this case, the surgeon will put a tube through the patient’s bladder and up into the kidney, and will use X-ray imaging to make a puncture in the kidney so that the stones can be removed. If the patient has very large stones, the surgeon may need to make more than one puncture.
When the surgeon has access to the kidney stones, he or she will place a sheath into the kidney and break up the stones using a special instrument. If the fragments are small enough, the surgeon will remove them. When the procedure is finished, the surgeon may leave drainage tubes in the kidney. Your surgeon may send the kidney stone fragments to be analyzed or to check for infection.
You will be asked to stay in hospital for one or two days after the treatment. Your doctor will recommend avoiding heavy lifting, and pushing or pulling for two to four weeks. You may be able to return back to normal routine after a week.
The most common risks of a Percutaneous Nephrolithotomy include the following:
