Brain Aneursym Surgery

DESCRIPTION

A brain aneurysm surgery is a bulge in a blood vessel caused by a weakness in the blood vessel wall, usually where it branches. It is a procedure to prevent an aneurysm from rupturing or to treat a ruptured aneurysm. This occurs when there is a blood vessel bulging because of a weakened wall. Because of the weak blood vessel wall, blood flowing through the wall applies pressure on it, which results in swelling and a bulge.

Though an aneurysm can occur anywhere in the brain, it is most likely found in the base. If the doctor suspects an aneurysm, an MRI or a CT scan is recommended to confirm the diagnosis. In most of the cases, an MRI is advised when the aneurysm has not ruptured while a CT scan is preferred when it is already ruptured.

This treatment depends on whether it has ruptured or not. In case it has not ruptured and is not at risk, doctors might just prescribe medications and lifestyle changes to lower the blood pressure. INDICATIONS

There might not be any symptoms in the body if the aneurysm has not ruptured or leaked. Many people, in fact, go years without realizing the aneurysm in the brain. However, if the aneurysm leaks, it causes brain haemorrhage and showcases symptoms like:

  • Impaired speech or double vision

PRE-PROCEDURE

Since this procedure is conducted under general anesthesia, the doctor advises the patient to stop eating or drinking for at least 8-10 hours before the surgery. DURING PROCEDURE
To repair the brain aneurysm there are two surgical approaches. One involves surgically clipping the aneurysm while the other involves using an endovascular coil to close off the artery. Both procedures are performed under general anesthesia by a highly skilled neurosurgeon and his team.

Surgical Clipping – This procedure is followed by making an incision in the scalp and a part of the skull bone is removed to gain access to the brain. Then the neurosurgeon will apply a metal clip on the aneurysm to close it and leaves it there for permanently closing the aneurysm and preventing it from rupturing again. Then, the skull bone is placed and the incision is carefully closed with sutures.

Endovascular Coiling – This procedure is a minimally invasive approach that involves inserting a catheter into an artery in the body (arms or legs) and then guiding it to the blood vessels into the brain and the aneurysm. Platinum coils are passed through this catheter and released at the aneurysm. This can cause clotting, thus preventing blood from entering into the aneurysm. Surgeons can close the aneurysm from the artery and prevent it from rupturing again using this approach.

POST PROCEDURE
Post brain aneurysm surgery, the patient is moved into an ICU and closely monitored. The patient needs to stay in the hospital just a few days if there is no bleeding in the brain before the surgery. Otherwise, you might have to stay for a week or two in case any complications occur in the surgery. It usually takes longer time to recover for the patients who undergo a clipping surgery than those who get an endovascular aneurysm repair.

Clipped aneurysm takes more time to heal and thus, patients must take it easy for at least 4 to 6 weeks post the surgery and only resume daily activities after their doctor confirms everything is okay.

An endovascular patient can resume activities within a few days of the surgery. However, in the case of endovascular repair, there are more chances of the aneurysm to reoccur and therefore, the patient needs to constantly go for follow-up angiography to keep monitoring the aneurysm.
